Loaning Samples for a Shoot

Anyone have any sample agreements/documents to simply state terms and conditions about loaning clothing samples for shoots? I've got a modeling agency that wants to borrow some of my pieces to shoot new faces. No big deal. I'd just like to write up an agreement that states basic conditions like payment for damaged, lost items, dry-cleaning for stains etc. Anyone have any ideas or a template I could work off of? Thanks a lot
